2. Swim Card Form

   Parent/guardian: Complete this waiver if your child ages 12-17 wants to swim during non-lifeguarded hours. An advanced swim test is required—details are provided below.

   After your complete this form, our aquatics staff will contact you to schedule the child's advanced swim test.
   
   Note: This policy was revised in March 2025. Previously, ages 10-17 could take the advanced swim test, but the age requirement has now changed to 12-17.

27. Parent/Guardian Agreement

    I understand:

    • My child must take and pass an advanced swim test
    • I must be on the pool deck to supervise my child while they swim
    • I am aware of and accept the risk of using the pool while there is no lifeguard on duty
    • My child must display their swim card on the pool deck while practicing swimming
    • My child must be actively circle swimming while in the pool
    • This is a time for fitness lap swimming and not individual coaching sessions
28. Advanced Swim Test

    The advanced swim test includes:

    • 100 meter swim (stroke of your choice) without interruption
    • One minute treading water in the deep end of the pool
    • Our swim test can take up to 10 minutes
32. Hold Harmless Agreement

    I hereby state that I am aware of and accept the risks inherent in this activity. The undersigned does hereby agree to hold harmless and indemnify the Veterans Memorial Community Center, their offices, agents, and employees from any and all liability, loss, damage, costs, or expenses which are sustained, incurred, or required arising out of the actions of my dependent in the course of swimming during unguarded hours. All liability for injury and other claims is forfeited by the participant against the City of Inver Grove Heights or any personnel involved with it.

    By signing this waiver agreement, the signee has reviewed and agrees to the pool policies and that all information is correct to the best of their knowledge and belief.
